Health Ministries Clinic is proud to support the community by offering FREE sports physicals during scheduled events ONLY in May, helping students meet their requirements without barriers.

These events are coordinated with each school—contact your school for details!

Both KSHSAA forms and the parental consent if applicable are required for all sports physical events in order to be completed!

Appointments ONLY: No walk-ins accepted.

Form Handling: Return all completed forms to your school; HMC keeps only a portion of the KSHSAA form.

Parental Presence:

• Ages 12 & under: A parent/guardian must be present.
• Ages 13 & older: A signed consent form allows the exam without a parent/guardian present.
