Sports Physicals/Clinics

For any athlete who plans to participate in athletics, sports physical clinics/appointments are offered at the following locations.
Physicals by Appointment. Some clinics may provide walk-in hours.
Please confirm fees & appointment/walk-in schedules with provider.

You must print a KSHSAA PPE (Pre-Participation Physical) form before you go to have the physical completed.
You must have a completed physical BEFORE you can participate.
